ELIGIBILITY:
 

• Candidate must be current high school senior planning on attending a post-secondary institution.

• Candidate must have demonstrated exceptional academic achievement.

• Candidate must have significant involvement in school and community activities.

• Candidate must have participated in a high school athletic program.
   

APPLICATION REQUIREMENTS:
 

• Attached application must be submitted to contact@2ndphasefoundation.org

• The candidate will submit an essay describing the impact sports has had on your life and a brief summary on their career goals. (Minimum 400 words)

• High School Transcript

• With application candidate will submit

  • At least one letter must be from a previous coach. The letter should speak to the character, integrity and or uniqueness of the student.
     

ADDITIONAL INFORMATION:
 

• Applications will be accepted by email only.

• Finalists will be interviewed by the selection committee in May.

• Scholarship will be presented in May(date TBD).

• The winner will receive the scholarship award once college acceptance and enrollment is verified by the 2nd Phase Foundation.

• EDUCATION POSTPONEMENT: If the winner chooses to postpone his/her college education, the scholarship may be reserved for up to one year from the date of graduation.
